At Hospice of the Golden Isles, we provide hospice care in the comfort of your home, wherever you live, including a private residence, assisted living facility or nursing home, hospital, or our Hospice House.
Regardless of where you live, members of our team will provide expert hospice care, offer caregiver assistance and education, serve as liaisons between you and your physicians, provide medications and medical equipment, and work with your loved ones to help them feel confident as caregivers.
Our hospice staff is available 24 hours a day, seven days a week, to help with questions and concerns.
Most hospice patients receive care in their own home. Typically, a family member serves as the primary caregiver and, when appropriate, helps make decisions. Members of the Hospice Team make regular visits to assess patients and provide additional care or other services, and a Nurse Case Manager develops an individualized plan of care based on each patient’s goals and wishes. The Case Manager works with a team including our Chief Medical Officer, Nurses, Certified Nursing Assistants (CNA’s), Social Workers, Chaplains and trained Volunteers to ensure the best possible care.
We also provide services in nursing or assisted living facilities. Hospice offers our comprehensive interdisciplinary services at care facilities and works closely with the facility’s team to create and implement an individualized care plan. The Hospice Nurse Case Manager coordinates patient care and provides skilled pain assessment and medication management under the direction of the patient’s physician. In addition, Hospice CNA’s provide personal care to complement CNA services in these facilities.
In-home Crisis Care is also available for short periods of time in your home when skilled nursing care is required to help manage acute pain and symptoms.
