From the day hospice care begins, we bring our award-winning services to you, wherever you reside – at home, in a nursing facility, at a hospital, our Hospice House, or elsewhere. Our hospice care is delivered by an expert team of over 100 professionally trained, highly compassionate individuals, including physicians, nurses, certified nursing assistants, social workers, volunteers, chaplains, and bereavement counselors. We also offer caregiver relief.

Quick Tips For Symptom Relief

Our services include:

  • Expert, individualized care by an interdisciplinary team which includes a physician, nurse case manager, social worker, chaplain, hospice aides, volunteers and special therapists, as needed.
  • Prescription drugs and medical supplies related to the hospice diagnosis.
  • Durable medical equipment such as a hospital bed and oxygen.
  • On-call availability 24 hours a day, 7 days a week.
  • Medical oversight by a full-time, Board-Certified Hospice and Palliative Care physician.
  • Access to services at our Hospice House including short-term inpatient care, respite care, and residential care (based on availability).
  • Comprehensive Bereavement (grief and healing) services for our patients’ families and for others who want and need this special support in our Southeast Coastal Georgia community.
  • Pet therapy and pet visitation for patients in our Hospice House.
